Initial Consultation
We recognize the essentiality of the consultation in the care pathway. It entails a comprehensive examination of the anatomical, environmental, and medical causes of your distress. This information and necessary medical records and testing will yield a personalized treatment plan. Many people find that understanding the source of their problems imparts a new perspective and hope for the future.
An initial consultation takes 120 minutes. To make this short period productive, please gather your records and recollections of previous treatments before the first session.
Together you and Dr. Rama will review your current concerns and past conditions. Part of this process includes speaking with, if available, family members, bedmates, and previous care providers. Generally, you will spend half of the consultation period reviewing the nature of your problems, their origins, and the course of any previous treatment.
Fees
The initial evaluation includes a comprehensive sleep brain evaluation utilizing a brain map, rhinomanometry, capnography, and oral motor strength testing. We may recommendation additional testing, such as home sleep testing, laboratory sleep testing, blood tests, urinalysis, and a Cone Beam CT.
Often, we develop a team of specialized clinicians spanning multiple disciplines to collaborate with on your care. We work with psychiatrists, allergists, dentists, orthodontists, head and neck surgeons, and maxillofacial surgeons. The specialist's expertise and your specific needs will form the basis of the referral.
Our personalized treatments utilize the latest scientific innovations such as brain TMS, brain HBOT, myofunctional therapy, CBT for Insomnia, positive airway pressure, mandibular advancement splints, and CapnoTherapy.
An initial consultation entails a thorough review of history and examination, which can occur in the office, at your home, or via telehealth. The fee and type of assessment vary depending upon the appointment type.
The fee for an initial office visit is $800. This appointment lasts 120 minutes and includes a brain map, rhinomanometry, capnography, and oral muscle testing. .
The fee for an intial in-home appointment is $1500. This appointment lasts 120 minutes plus physician driving time.
The fee for a one-hour return appointment is $400.
Due to extensive testing, initial appointments are preferably in-person. An initial telehealth appointment can scheduled by request and not through the website.
We perform international consults via telehealth.
Insurance
Sleep and Brain is out-of-network for all insurance carriers. Contact your health insurance provider to determine if your care qualifies for reimbursement. We can provide you with a medical receipt known as a superbill at the end of each visit to seek insurance reimbursement. You are responsible for paying the fee in full at your appointment’s conclusion. The superbill, however, will allow you to request reimbursement for a portion of your cost from your insurance carrier, if your plan allows. The providers at Sleep and Brain do not accept Medicare and have signed an agreement to be excluded from the Medicare program. Consequently, Medicare will not pay for the care you receive.
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges. You have the right to receive a Good Faith Estimate explaining how much your health care will cost. The Good Faith Estimate shows the total expected cost of any non-emergency items or services and equipment. You may request a Good Faith Estimate in advance of an already scheduled health care service or item, or before scheduling an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy of your Good Faith Estimate. Payment Method
We accept cash, check, and card. We use Square to process cards. It works with credit, debit, HSA and FSA cards. It’s HIPAA-compliant, keeps our consultation confidential and makes payment easy for you and us. Payment is requested at the appointment time.
Cancellation Policy
We realize that life is unpredictable, and sometimes mix-ups occur; however, to ensure the service quality, we require a 24-hour notice for cancellations. A 100 dollar fee will be charged for appointments that are cancelled with less than 24 hours’ notice or missed initial appointment consultations.
